To establish an account with Longleaf, please fill out the applicable application below and send a copy of your State Tax Exemption Certificate. The purchases will be subject to state/local sales tax until we receive your specific state sales tax exemption certificate or your state's resale certificate, whichever is applicable.  We will not credit for state sales tax if it reflected on the invoice. If the purchases are not exempt from these taxes, please notify us. • For a Line of Credit:  Please email, fax or mail Credit Application form (PDF download) along with a copy of your State Tax Exemption certificate, if applicable.
• For Prepay Accounts:  Please email, fax or mail Prepay Information Sheet form (PDF download) along with a copy of your State Tax Exemption certificate, if applicable.
• For Federal and State Government Agencies only: Please email, fax or mail Government Application form (PDF download) along with a copy of your State Tax Exemption certificate, if applicable.

Orders for all publishers can be combined on one PO and will be combined for shipping. The preferred method for ordering is PUBNET for all publishers (SAN 203-3151).

If this is a one-time purchase and you do not wish to establish an ongoing account with Longleaf, we will be happy to process your order by prepayment with American Express, Discover, MasterCard, Visa, or USD business check. The purchases will be subject to state/local sales tax until we receive your specific state sales tax exemption certificate or your state's resale certificate, whichever is applicable.
